Studies have shown that adolescents in the Teen Outreach Program are at 52% lower risk of school suspension, 60% lower risk of course failure, and 53% lower risk of teenage pregnancy.

To facilitate behavior change among overweight adults that leads to sustained weight loss.
Those who use Text4Diet are exposed to customized advice regarding healthy behavior changes. Participants on average saw an average 6-pound weight loss during a four-month use period. This is a statistically significant difference from weight loss in the usual care comparison group.

The goal of this intervention is to reduce high-risk behavior among African American youth as measured by student self-reports of violence, provocative behavior, school delinquency, substance use, and sexual behaviors (intercourse and condom use).
AAYP reduced rates of risky behaviors among male African American youth.

The Ambulatory Integration of the Medical and Social (AIMS) model aims to address social and environmental factors patients face that may prevent them from following their plan of care, thus impacting their health.
The AIMS model helps create better supported, less stressed, and better informed consumers and caregivers. There is also evidence to suggest that this model reduces ED usage and 30-day readmissions in participants.

The goal of this study was to determine the effect of interdisciplinary primary care teams on health care utilization by patients with multiple chronic conditions.
This study concluded that guided care models can significantly impact home health care episodes.

The mission of the Green House Project is to partner with organizations, advocates, and communities to lead the transformation of institutional long-term care by creating viable homes where elders and others enjoy excellent quality of life and quality of care.
Studies showed that Green House homes are likely to reduce hospitalization rates among residents, and Medicare and Medicaid costs per resident, when compared to residents in traditional nursing homes. Also, nurses are more likely to spend more hours in Green House homes.

The Incredible Years® Parents, Teachers, and Children Training Series has two long-range goals. The first goal is to develop comprehensive treatment programs for young children with early onset conduct problems. The second goal is the development of cost-effective, community-based, universal prevention programs that all families and teachers of young children can use to promote social competence and to prevent children from developing conduct problems in the first place.
Studies have shown that children who participate in the programs demonstrate significant improvements in school readiness, emotional regulation, and social skills, as well as reductions in behavior problems in the classroom.

The Living Room is a comfortable, non-clinical space that offers an alternative to hospital emergency rooms. The program in LaGrange, Illinois hopes to decrease unnecessary hospitalizations.

GAIN is part of a large-scale, welfare-to-work initiative program operating in every county in California. In L.A. County, the initiative is under the supervision of the Department of Public Social Services. It helps local businesses and employers find and hire quality workers who seek meaningful employment. Prospective workers are participants in the state welfare programs known as California Work Opportunities and Responsibility to Kids (CalWORKS) or General Relief Opportunities for Work (GROW).

The proportion of URM applicants to MUSC psychiatry residencies increased from 8% in 2008 to 17% in 2015. The percentage of URM psychiatry residents enrolled in MUSC has tripled over the last seven years, from 9% in 2008 to 28% in 2015.
